A root tip resection with orthograde filling is a surgical removal of the root tip and the cleaning of the dental canal crown-wards. Like the root treatment the RTR pursues the highest goal which is to clean the root canal. Actually this should happen during a root treatment. Unfortunately this is not invariably the case and thus a RTR is the result. If the root canal is accessible from above it has to be cleaned from above. If the center of inflammation on the root tip is too big, a bit of the root tip will also be removed.
