In the 2017 Familial hypercholesterolaemia guidelines, NICE suggests we periodically search people aged 30 years or older with a total cholesterol concentration greater than 9.0 mmol/L to consider a referral to the lipid clinics.

There are two ways to create a search, using the parent child search format or through a direct search. I will show you the parent child format, because I feel it better illustrates things. Click on the EMIS ball in the top left hand corner and click reporting - population reporting. These are your searches. Create a folder with your name, and then a folder with your search.
Cholesterol greater than 9.0 in over 30 year olds

Then click add - patient - search

Now you can see here that there are two cholesterol options, if you want to be sure you are searching for the right one, find a total cholesterol value in your patient’s notes, click on SNOMED code and copy the ID and then paste it when searching.

In Adults: REFER:

1. Suspect familial hypercholesterolaemia (FH) in adults with:

- A total cholesterol level greater than 7.5 mmol/L or LDL more than 4.9
AND
- 1st degree relative with MI aged less than 60 or 2nd degree relative aged less than 50
OR
- High cholesterol in 1st/2nd degree relative more than 7.5mmol/L


2. If LDL more than 8.5 – refer regardless of family history (Dutch Lipid Clinic Network Criteria)


3. If TG more than 4.5 – repeat fasting: refer if

a. If TG more than 4.5 and LDL more than 7.5
b. TG more than 10

You can also export this list by clicking on patient list, edit it and run the report. You can rename it as well then click on view results and now you can export that in Excel and edit the spreadsheet as required. You can also access the patient notes directly from the search itself. And that’s your quality improvement done! Finally, if you wanted to re-audit, you could use another rule – exclude patients that have the code that you entered where you were reviewing the notes. – e.g. serum cholesterol very high. Add the code then choose exclude patients.

Finally, I promised you a bonus at the end, here’s a super fast way to load up a new consult using two function keys – F6 and F4. Normally if you wanted to load up a consultation, you would go to your appointment book, double click on the patient, click on the consultation icon and then click add. But a faster way to do this is double click on the patient and then click F6 F4 and there you have it!
