Act 3 Hellfire Greataxe
First up is the Hellfire Greataxe. This axe is very rare and deals 1d6 additional fire damage, afflicts the wielder with heat on each successful hit through thermodynamo, and grants Hellflame Cleave which deals an additional 2d6 fire damage which ignores resistance and immunity to fire damage.

This powerful axe is hidden in a very clever way. You’ll need to head to the vault within Sorcerous Sundries and go through the "Silverhand"  door. In the next area you’ll see a door called "Illusion". Well instead of clicking on it, you need to quite literally smash it down. And behind the “Illusion” door is a chest containing this amazing great axe. I thought this was a very awesome way to hide an item.

Act 2 Potent Robe
Number 2 is the Potent Robe. This robe is the best in the game for Warlocks relying primarily on Eldritch Blast - it grants Gregarious Caster causing your cantrips to deal additional damage equal to your Charisma Modifier - AND it grants Well-Liked and Well-Fortified which grants temporary hit points equal to your charisma modifier at the beginning of the wearer’s turn. Warlocks being a class that tries to max out their charisma it is really the perfect robe for them.

To get this robe you need to complete the quest Rescue the Tieflings in Act Two, and Alfira will give this to you.

This means you need to save the grove in act 1 AND ensure that Alfira in particular DOES NOT DIE. Without spoiling anything, this robe is extremely easy to miss if you’re doing a Dark Urge playthrough.

The quest involves infiltrating Moonrise towers and breaking the remaining tieflings out of prison.

So if you’re a warlock, especially a Dark Urge Warlock, I’d highly recommend playing intentionally in order to get these amazing robes!

Act 1 Intransigent Warhammer
Number 3 is the Intransigent Warhammer. This uncommon warhammer is versatile and knocks nearby foes Prone after killing a hostile target, or landing a critical hit. And while this warhammer might not be the strongest in the game, it sure is easy to miss - you have to initiate a fight on the Duergar Boat in the underdark as you travel to the Gyrmforge, and after winning the battle, you’ll need to search the enemy’s vessel for a chest where you will find this warhammer. Knocking enemies prone can enable certain builds that rely on it, so it does have its uses.

Act 1 Hamarhraft
Number 4 is the Hamarhraft.
This maul deals 2d6 + your strength modifier in bludgeoning damage. But very interestingly, it also provides shockwave which makes the wearer deal 1d4 thunder damage in a 3 meter radius upon landing after a jump. This has led to ridiculous builds where you just jump repeatedly or “stomp” your enemies to death. So this hammer unlocks a very unique playstyle.

To find this strange maul, you’ll need to rush into the burning building at Waukeen’s rest before it becomes completely engulfed in flames. Within the burning building you’ll find a gilded chest on the second story and the maul lies within it.

Act 3 Staff of Cherished Necromancy
Number 5 is the Staff of Cherished Necromancy. This staff is the best in the game for necromancers because it causes targets of your necromancy spells to have disadvantage on saving throws, and most importantly Life Essence Harvest. This causes you to gain Life Essence after killing a hostile creature with a spell until long rest. Now I’m not sure if this isn’t working as intended, but it appears if you kill just one enemy, you can now cast all your necromancy spells without using a spell slot until your next long rest. So this staff enables endless Necromancy spells to be cast. Absolutely amazing.

The only way to get this staff is by looting the corpse of Mystic Carrion in Philgrave’s Mansion in the Lower City. Even though there is a quest to determine how to truly kill Mystic Carrion you can slay Mystic Carrion the good ol fashioned way and loot his corpse for this staff. And I’ve got to say, this staff is pretty mind blowing.
