Salter-Harris Fractures

Salter-Harris fractures are growth plate injuries that occur in children and adolescents. These fractures are particularly important due to the potential for growth disturbance and subsequent deformity. The Salter-Harris classification system utilizes the mnemonic "SALTeR" to categorize these fractures based on the fracture pattern and involvement of the growth plate:

Type 1 (S: Slipped or separated):
Salter-Harris type 1 is a simple, transverse fracture through the growth plate with no involvement of the metaphysis or epiphysis.

Type 2 (A: Above):
Salter-Harris type 2 is a fracture that extends from the growth plate into the metaphysis, leaving the epiphysis and a portion of the growth plate intact. Type 2 fractures are most common and occur away from the joint space.

Type 3 (L: Lower or beLow):
Salter-Harris type 3 is a fracture that extends from the growth plate into the epiphysis, leaving the metaphysis and a portion of the growth plate intact.

Type 4 (T: Through):
Salter-Harris type 4 is a fracture that extends through the metaphysis, growth plate, and epiphysis. Types 3 and 4 fractures each carry a risk for growth retardation, altered joint mechanics, and functional impairment.

Type 5 (R: Raammed or cRush): Salter-Harris type 5 is a crush or compression injury to the growth plate. Type 5 injuries typically have a poor prognosis leading to bone growth arrest.
