Upper Occlusal (Michigan) Splint for treatment of Facial Athromyalgia

Provision of an upper hard acrylic occlusal splint, constructed on a denar semiadjustable articulator, for the treatment of Facial Athromyalgia or temporo-mandibular joint dysfunction syndrome (TMD) or Myo-facial pain dysfunction sydrome (MFPDS). Young adult presented with mild increased incisal wear and facial athromyalgia caused by dental parafunctioning. Incisal wear was restored with direct dahl composites. After a period of stabilisation a michigan hard acrylic occlusal splint was constructed using a Denar semi-adjustable articulator. A flat posterior bite plane was produced on the splint with light holding contacts in centric relation at all teeth and no lateral functional guidance on the posterior teeth in either lateral or anterior excursions. A shallow incisal/canine ramp was created on the splint to produce posterior disclusion during anterior and lateral excursions. The splint also helps to diminish  proprioceptive feed-back from the teeth and is thought to encourage the mandible to retreat into a more relaxed retruded position.The patient was recommended to wear the splint regularly at night. The occlusal splint should also help to prevent further tooth surface loss (TSL) caused by attrition during dental parafunctioning and also reduce the progression of cervical abfraction lesions.  Studies show that maximum benefit is gained from occlusal splints with long term use although an improvement in symptoms is generally seen quite quickly within a few weeks of regular use.
