About the CQI and IRCA
The CQI is the only chartered professional body dedicated entirely to quality. IRCA is its specialist division dedicated to management system auditors.

Occupational Health and Safety (OH&S) Management System

Communicating during the Audit- The audit team needs to confer periodically during the audit, the reasons for this include
To exchange information
Assess Audit progress
Re-assign work between audit team members as may be required

Communication with the auditee and audit client should be consistent throughout the audit. Concerns should be raised with auditee/audit client as they are seen, if audit evidence suggests immediate risks these should be reported to auditee and as appropriate the audit client immediately. Concerns outside the audit scope can be noted for the attention of the auditee and audit client.
It is possible that available audit evidence indicates that it is impossible to achieve audit objectives; the team leader should work in concert with auditee and audit client to determine a course of action which could include

Modification of audit plan
Changes to audit objectives
Changes to audit scope
Termination of the audit

Where there are changes to audit plans as audit activities progress these should be agreed and approved immediately by key stakeholders.
Assigning Roles and Responsibilities of Guides and Observers- Guides and observers should not influence or interfere with the conduct of an audit. The lead auditor decides with due consideration to stakeholders if observers and guides should be involved in an audit.  Guides are personnel of the auditee that are selected to assist the audit team in navigating their way through the organisation. An auditor should always have a guide at any time during an audit.

The roles of a guide are detailed above.

Observers are generally to be seen and not heard. However depending on the context of the audit e.g. a second party audit where an observer is representing the audit client, there potentially could be some exception to this rule.

Collecting and verifying Information- Only verifiable information can be accepted as audit evidence. Below is an overview of the process of collecting and verifying information as provided by ISO 19011:2018.

Various methods can be utilised in collecting information, these range of tools available to the auditor are called Audit Methods - see section on Audit Methods.
There are many sources from which information may be obtained by an auditor including but not limited to

Interviews with employees and other interested parties
Observation of activities and surrounding work environment and conditions
Maintained Documented Information- such as policies, objectives, plans, procedures, standards, instructions, licenses, permits, specifications, drawings, contracts*.
Retained Documented Information e.g. inspection records, minutes of meetings, audit reports, records of monitoring and results of measurement, orders.
Data summaries, analysis and performance indicators
Information on auditee sampling plans and on procedures for the control of sampling and measurement
Reports from other sources e.g. customer feedback, external surveys and measurements, other relevant information from external parties
Databases and websites
Simulation and modelling
*Depending on context the italicised could be maintained or retained documented information
