Epoetin alfa is classified as an erythropoietin-stimulating agent. It is used to treat drug-induced and disease-related anemia, like chronic kidney disease and cancer.
This medication works by stimulating division and differentiation of erythroid progenitor cells, which increase the development of mature red blood cells.
Main adverse effects include Hypertension, bone pain, pure red cell aplasia, and cutaneous reactions, like Stevens-Johnson syndrome.
Boxed warnings: Epoetin alfa has increased risk of cardiovascular events like stroke, myocardial infarction (MI), and venous thromboembolism (VTE) if hemoglobin level is greater than 11g/dL. Additionally, if the hemoglobin level exceeds 10 g/dL, doses of epoetin alfa should be reduced or treatment should be discontinued.
In cancer patients it can speed up tumor progression, risk of recurrence, and decrease chances of survival. Epoetin alfa is only effective if adequate iron is available to make hemoglobin. So, iron supplementation may be required to ensure an adequate response.
