Syphilis is a sexually transmitted infection (STI) caused by the Treponema pallidum bacterium. T. pallidum is a spirochete that cannot survive outside of the human body for long periods of time.
Transmission occurs from direct contact with infectious lesions when the bacteria enter the body through the mucous membranes or skin, usually during sexual contact. Syphilis can also spread from a mother with syphilis to her unborn baby causing congenital syphilis. Within hours after entering the body, the bacteria reach the regional lymph nodes and rapidly spreads throughout the body.
Signs & Symptoms of Syphilis
Primary syphilis
• Characterized by chancre or syphilitic sore, which is single or multiple, painless, indurated, open sore
• Chancres resolve within 3 to 6 weeks
• Lymphadenopathy or swollen lymph node that is regional to the site of the chancre

Secondary syphilis
The symptoms of secondary syphilis are multisystemic
This stage usually begins with a rash on one or more areas of the body
• Alopecia
• Sore throat
• Fever
• Headache
• Swollen lymph nodes
• Muscle aches
• Fatigue
• Unexplained weight loss

Latent syphilis
No visible symptoms, but the bacteria continue to multiply and invade the organs

Tertiary syphilis
Tertiary stage is the late stage of syphilis and symptoms would occur 10 to 30 years after your infection began
              Cardiovascular syphilis
• Shortness of breath
• Palpitations
• Chest pain
• Heart failure

Gummatous syphilis
• Very uncommon
• Characterized by presence of Gumma, which are granulomatous nodules with central necrosis
• It can occur on any part of the body
Neurosyphilis syphilis
• Characterized by severe headache, muscle weakness and changes of mental state
Ocular syphilis
• Characterized by eye pain, redness and changes in your vision or even blindness
Otosyphilis
• Characterized by hearing loss, ringing, buzzing, roaring, or hissing in the ears also known as tinnitus and dizziness or vertigo

Diagnosis of Syphilis
Syphilis can be diagnosed by certain laboratory tests. Syphilis testing is usually done by a simple blood draw. If there are open sores, a sample of fluid from the sore may be swabbed and tested. Serologic treponemal tests for antibodies to T. pallidum is done.

Treatment of Syphilis
In the early stages of syphilis, treatment can be successful. Patients with syphilis will often receive an intramuscular injection of penicillin G benzathine during the primary, secondary, or tertiary stages. Multiple injections at weekly intervals are required for tertiary syphilis. To eradicate the bacteria from the central nervous system, neurosyphilis requires intravenous (IV) penicillin every 4 hours for 2 weeks. Always confirm the nature of the allergy in patients with a penicillin allergy. Alternative antibiotics such as doxycyline, ceftriaxone, azithromycin, or erythromycin can be administered in those who have a documented penicillin allergy.
