Google Form Creation for use with Scan to Web

You can use Scan to Web to fill a Google Sheet by way of a Google Form. The Google Form provides a clean interface to save data from a mobile device. Also it can be shared with multiple people.


1. On a computer (not your phone) open a web browser
2. https://drive.google.com
3. New + … More  … Google Forms … Blank Form
4. Click “Untitled form”; change to “Barcode Scanning”
5. Click “Untitled Question”
6. Change list from “Multiple Choice” to “Short Answer”
7. Click “Untitled Question”; change to “Barcode”
8. In upper right of screen, click big SEND button
9. In Send via, click the paper clip in the middle
10. Check the Shorten URL check box
11. Click COPY
12. Open another Web Browser or Web Browser tab
13. Go to our website https://berrywing.com/barcode
14. Click “What do you want in your barcode?”
15. PASTE the web address from step 11
16. Click the QR Code button
17. Scroll the screen to “Your Barcode” and leave this here.

18. On your phone, tablet, or mobile device with the Scan to Web app
19. Tap the GEAR button on the bottom toolbar
20. Tap the QR Code button ABOVE “Home Page URL:”
21. Scan the barcode from step 17. Above
22. Confirm the Home Page URL matches the web address on the screen in step 17.
23. Tap the BACK button
24.  Tap the HOME button to load the new home page

Your Google Form is now ready to scan barcodes into.
