1. Sign in to your Chase account, choose “Investments,” then “Summary”
2. Choose “Things you can do,” then “Services”
3. Choose “Provide Cost Basis” to make an update
4. Choose the account for which you’d like to request a cost basis update
5. Enter the security description and symbol or CUSIP for this security
6. Choose the date when it was purchased
7. Enter how many shares you purchased
8. Enter the total purchase amount
9. You can add another cost basis (up to 10 on the same request), or choose “Next”
10. Review your information; choose “Change” to make any edits, or choose “Submit”
11. From here, you can add another cost basis or choose “Done”
12. We’ll send you a secure message with updates, which you can find under the main menu
