Me playing the last level of Quake 3 Arena against Xaero on Nightmare skill with no deaths, 10 times in a row.

Xaero can be defeated with no deaths quite consistently. You need to have good judgement of which side he will respawn on next: this can usually be determined based on where he was last killed.
If he is going to respawn on the other side, then you can kill him easily by peeking around the pillars and landing two railgun shots. If you narrowly peek around a pillar, you can shoot at him without him trying to shoot back.
If he is going to respawn on your side, then you should:
* Pick up the railgun pickup so it is on cooldown when he respawns, and thus not available to him.
* Spawn camp one of the respawn points (I pick the one nearest to the railgun pickup).
* (if you have the BFG) Use his respawn sound to determine which spawn point he is at, and spam BFG shots at the appropriate location to quickly finish him off.
* (if you only have the railgun) Anticipate the path he will take after he spawns (usually he walks to get the rocket launcher, because the railgun is not available), and lie in wait to make railgun shots at the path. Try to time it so that he comes into view at about the same time as when your next rail is ready. There are only about five common initial patterns based on which spawn point he respawns at.

I played half of the games using a railgun-only strategy, and the rest using a mixture of the railgun and the BFG. Once you have the BFG, killing Xaero when he respawns on your side obviously becomes easier, but getting the BFG in the first place is a risk, because Xaero sometimes shoots you out of the air with his railgun as you take the jump pads up to the BFG platform. I am still unsure as to which strategy is easier to achieve deathless with overall.

In each of game 2 (1m17s) and game 8 (7m40s), I fire one seemingly pointless railgun shot because the game missed the previous mouse-up event. In game 6 (4m59s), the railgun similarly fails to fire (while I am flying to the BFG platform and zoomed in with Xaero in the crosshairs) because the game missed the mouse click event.

In game 2, as I jump down from the BFG platform (1m14s), I unconditionally spam the BFG at the railgun pickup because if Xaero respawns next to it, I would like him to be promptly killed, else he might pick the railgun up and rail me out of the air as I fall down.

After I decided to try to record a 10-streak, I played about 196 games before I got this video. I attribute the poor success rate to the video capture hurting the frame rate a lot (which took some time to get used to) and causing Quake 3 to drop mouse inputs (which you can never get used to).
