What do you need to obtain your first U.S. Passport?
1. A copy of the DS-11 form, the application for a US Passport, found here:
http://www.state.gov/documents/organi...
2. Original, certified birth certificate or naturalization certificate
3. Two passport-sized photos, 2"x2"
4. Check for $170 made out to the US Department of State (if you're over 16 years old)
5. Two original and signed passport authorization forms, found here:

6. Evidence of immediate travel
7. Proof of identity
