Washes hands.
Explains procedure to resident. Speaks clearly, slowly, and directly. Maintains face-to-face contact whenever possible.
Provides privacy.
Positions wheelchair at right angle to the toilet to face hand bar on resident’s stronger side.
Removes footrests. Locks wheels.
Puts on gloves.
Applies a transfer belt over clothing. Grasps the belt, asking resident to push against armrests and to stand.
Asks resident to pivot and back up to feel front of toilet with back of legs.
Helps resident pull down pants and underwear.
Helps resident sit down slowly.
Removes and discards gloves. Washes hands, places call light within resident’s reach and leaves bathroom.
When called, returns and applies clean gloves. Assists with perineal care as necessary.
Cleans and dries resident. Removes and discards gloves. Pulls up clothing.
Helps resident into wheelchair and replaces footrests.
Helps resident to sink to wash hands. Washes own hands.
Helps resident leave bathroom. Removes gait belt.
Places call light within resident’s reach.
Washes hands.
Documents procedure.
