You can do the "Flawless
You can do the "Flawless Penance" achievement quite easily with the last boss (Incarnate Devotion). The method involves cheesing the boss with the "Seguiriya to the Memory of Your Eyes" prayer and landing hits using "Veredicto" while time is stopped. It will take about 9-10 prayer casts to defeat the boss. By using the altarpieces listed below, you can regain enough fervor to cast the prayer again while the previous prayer's effect is still active. The boss will hardly be able to cast any spells to damage you.
Make sure you have no guilt and use all of your bile flasks before starting the fight.
List of required altarpiece pairs:
The Traitor - Castula,
The Scribe - The Anointed One,
Tirso - The Thirst,
The Selfless Father - The Purified One (or any other of the same type)

Works on game version 1.0.5.
Since version 1.1.0 this prayer is active for only 3.5 seconds instead of 6 making it harder to cheese the boss.
