What happens during persistent versus paroxysmal atrial fibrillation (AFib). To learn more about how atrial fibrillation becomes persistent, watch this video.

Atrial stretch has been implicated in atrial remodeling. There is a mismatch in compliance between the heart and the pericardial reflections seen here as white lines. The reflections do not stretch and thus the heart is under mechanical stress. Added factors like heart disease or obesity significantly increase the mechanical stresses along the reflections.

These high stresses cause the left atrium to stretch and enlarge. This can lead to inflammation, collagen deposition and fibrosis seen here as dark patches on the posterior wall. Over time fibrosis becomes extensive, covering a substantial portion of the posterior in response to fibrosis abnormal substrates form seen here in blue. These substrates give rise to arrhythmias and the majority of substrates may be located along the posterior wall.

Atrial fibrillation (AFib) is an abnormal heart rhythm that arises from excitable electrical tissue in the left atrial chamber or pulmonary veins. Blood enters into the Left Atrium from the lungs, through four pulmonary veins. The lining of the left atrium spreads into the pulmonary veins, and this lining is electrical in nature. As the left atrium or the pulmonary veins are stretched or enlarged by physical stress, the exposed nerve endings can fire out of turn like an exposed electrical wire whose insulation is damaged. This results in a sudden, erratic, irregular, and often fast heart beat, known as atrial fibrillation.
