This video shows you how to unlock the secret ending in Returnal.

Once you've completed the game once, begin a another run to locate and collect each of the Sunface Fragments. There's one located randomly in each biome, which you can only access with the equipment discovered by beating the game.
Once you've found all six fragments, return to the house and explore it once last time to receive a new key. After awaking outside, complete the run (or start another) and defeat Orphion one more time in the Abyssal Scar. You can then use the key on the car (which you passed at the end of your previous run) to see the secret ending.
