श्रमिक कार्ड की छात्रवृति योजना के लिए आवश्यक प्रमाण पत्र
1.श्रमिक का श्रमिक कार्ड की एक प्रति।
2.श्रमिक का बैंक खाते की पास बुक का प्रथम पृष्ट (श्रमिक का नाम , आई.एफ़.एस. सी कोड 3.अंकित हो ) की एक प्रति।
4.आधार कार्ड एवं भामाशाह कार्ड /जन आधार की प्रति|
5.जिस कक्षा या कोर्स के लिए छात्रवृत्ति चाही गई है, उसकी अंक तालिका की स्वप्रमाणित प्रति।
6.शिक्षण /प्रशिक्षण संस्था के प्रधान द्वारा प्रपत्र के निर्धारित कॉलम में हस्ताक्षर एवं मुहर लगाया जाना आवयशक है।

1. A copy of the labor card of the worker.
2. A copy of the first page of the pass book of the worker's bank account (name of the worker, IFSC Code to be numbered 3.)
4. Copy of Aadhar Card and Bhamashah Card / Jan Aadhar.
5. Self attested copy of mark sheet of the class or course for which scholarship is sought.
6. It is necessary to sign and stamp the prescribed column of the form by the head of the teaching / training institution.
