Dr. Keith Forwith is a board certified specialist with Advanced ENT & Allergy in Louisville KY.  He obtained his medical degree from Case Western Reserve University in Cleveland, a PhD in biochemistry from Purdue University and general and advanced surgical training at the University of Cincinnati Medical Center and Children's Hospital in Cincinnati.
