The body’s vestibular system maintains sense of balance. Vestibular disease can cause dizziness or vertigo and is diagnosed using head impulse testing, where an examiner rapidly rotates the patient’s head while the patient fixates on a target. When vestibular function is impaired, eye tracking fails during the head movement, requiring a catch-up eye movement (a saccade) to reacquire the target at the end of the movement. This slow-motion video (240 frames per second) demonstrates catch-up saccades bilaterally in two patients: a woman in her 60s evaluated for constant dizziness and multiple falls who had normal video head impulse testing (vHIT) findings, and in a man in his 60s evaluated for vertigo with acceleration and deceleration who had normal video head impulse testing (vHIT) findings.
