I did this in Chapter 3, at the beginning of the boss fight with Karai. The attack variations are the ones you practice in the Dojo back at the Turtles' HQ (headquarters). You can switch turtles during the combo but you have to make sure to be quick or else the combo will drop. 12 attack variations in one single combo and the achievement / trophy will unlock.
