9compartments of the foot:
Lateral
Medial
Two central: superficial and deep
Adductor hallucis
4 interosseous.
Compartment syndrome of the foot occurs from high energy trauma.
Clinical presentation:
Swelling of the dorsum of the foot.
Tenderness of the dorsum of the foot.
Pain with passive flexion or extension of the metatarsophalangeal joint.
Intracompartmental pressure is measured. Once compartmental pressures become elevated, fasciotomy is the treatment for compartment syndrome.
30+ mmHg or within mmHg of the diastolic pressure.
Fasciotomy:
two dorsal incisions centered over the 2nd and 4th metatarsals.
Medial incision is made to release the medial and central compartments of the foot.
