Multifocal lenses is not like bifocal glasses. Bifocal glasses that you place in front of the eyes have got a distant glass for seeing long distance and a small segment for viewing near objects. Whereas multifocal lenses are specialized designed optic which has got images of all the distances namely the distance, the intermediate and the near. The brain need to adjust to see what is its task. It is like all the images are present at the same time but the brain needs to suppress or ignore the other images. The analogy is something like music is being played in the background, but the music is not heard, but one can hear the conversation. In the similar way, the multifocal lenses, all the images are present at the same time. Implanting a multifocal lens bilaterally, one needs to properly counsel these patients. One needs to look at the patient’s expectations, patient’s motivations, whether the patient will accept these types of lenses because  unilaterally placed multifocal lenses take a long time to adjust because the brain has to learn to ignore certain images, Young patients who have got unilateral cataract are unlikely to develop a cataract in the other eye at an early date may not be happy with a unilateral multifocal lenses. Whereas patients who are at the presbiopic age or what is at the age where they require glasses for reading. They may appreciate the multifocal lenses better unilaterally or a patient who has got a cataract in the other eye, who will require a surgery in the near  future. In these patients implanting a multifocal lens unilaterally will be useful. But per se if multifocal lenses is implanted unilaterally, patient must be really motivated and will take time to adjust to these new lenses.
