In this video, Dr. Webb talks about the two main types of surgeries that we do as spine surgeons are: spinal decompression versus spinal fusion.

1. 𝐒𝐩𝐢𝐧𝐚𝐥 𝐃𝐞𝐜𝐨𝐦𝐩𝐫𝐞𝐬𝐬𝐢𝐨𝐧: Spinal decompression is a surgical procedure performed to relieve pressure on the spinal cord or nerve roots. It aims to create more space within the spinal canal, reducing compression and improving symptoms. This procedure is typically done to treat conditions like spinal stenosis, herniated discs, or degenerative disc disease.

During spinal decompression, the surgeon may remove a portion of a damaged disc, bone spurs, or any other structures that are causing compression. This can be done through different techniques, such as laminectomy or discectomy, depending on the specific needs of the patient.

2. 𝐒𝐩𝐢𝐧𝐚𝐥 𝐅𝐮𝐬𝐢𝐨𝐧: Spinal fusion is a surgical procedure that involves joining two or more vertebrae together using bone grafts, implants, or other devices. The primary goal of spinal fusion is to stabilize the spine and eliminate motion at a particular segment. It is commonly performed to treat conditions like spinal fractures, severe disc degeneration, spinal deformities, or instability.

During spinal fusion, the surgeon removes the damaged disc or portion of the vertebra and then fuses the adjacent vertebrae together. This fusion process promotes the growth of new bone, creating a solid bridge between the vertebrae. The fusion can be achieved using various techniques, such as using bone grafts, metal screws/plates/rods, or interbody cages.

It is important to note that both spinal decompression and fusion have their own benefits and potential risks. The choice of procedure depends on various factors, including the patient’s specific condition, symptoms, and medical history. It is always best to consult with a qualified spine surgeon to determine which treatment option is most suitable for an individual case.

In general, we try to avoid fusions if at all possible due to the problem of adjacent segment disease (ASD) - meaning the next level starts to break down after a fusion requiring additional surgery. If indicated, disc replacements can be used to minimize this downside of a fusion.
