Community Integrated Care is one
Community Integrated Care is one of the UK's largest health and social care charities. We work in the community, delivering life-enhancing support to people with a diverse range of care needs, including learning disabilities, mental health concerns, autism, age-related needs and dementia. We were founded in 1988 as one of the pioneers of the ‘Care in the Community Agenda’, with the aim of supporting people leaving long stay, institutionalised hospitals to lead fuller, better lives in the community. Since then, we have grown to provide a wide range of innovative and bespoke care services to over 5000 people living in England and Scotland.
Today, our charity is pursuing an ambitious strategy with the aim of becoming ‘The UK’s Leading Health and Social Care Charity’. This has seen us create ‘The Golden Thread’ a pioneering programme of investment in person-centred thinking. This has seen us invest in offering genuinely specialist training for all of our 5000+ colleagues; employ Peer Reviewers – people we support who are trained to assess the quality of life that people lead in our services; and launch an active Voice Forum, which enables people we support to contribute to our local and national decision-making.
We have also developed thriving local and national partnerships with care sector and community groups, to help enhance our services and sector, as well as offer new opportunities to our colleagues and the people we support. This includes us having an active involvement with national care and housing sector bodies such as National Care Forum, Association for Real Change (England and Scotland), VODG, ACEVO, SITRA and The Alliance. We also work closely with leading groups such as academic researchers, technology developers, football associations and creative groups, to develop exciting new programmes and initiatives.
Community Integrated Care is committed to providing genuinely integrated services. We have a long history of effectively bringing together NHS CCGs, local authority commissioners and housing providers to develop joined up services that enable people to lead better lives, whilst also reducing demand on local NHS and public services. We have been recognized for our success with this at the Skills for Care Accolades, in the ‘Best Approach to Integration and New Models of Care’ category.
We have been proud to be recognised for our high standards and ambitious approaches with hundreds of community and care sector awards, including at the National Dementia Awards, National Care Awards, 3rd Sector Care Awards, Laing Buisson Specialist Care Awards, and Scottish Care Awards.
