How to investigate the scene, in this incident investigation training video I want to discuss the steps a health and safety professional should be taking when they get to the incident scene. There are steps that need to be taken when investigating an incident that will help to collect the data in a practical manner. Following these simple steps will help the investigator when it comes time to form conclusions and make recommendations.
In this video we will review how to secure the scene, consider how to interview those involved and how the different elements of an incident scene need to be attended to.

• Where were you at the time of the incident? Might qualify with, in relation to other workers, objects, etc.
• What were you doing at the time? Duties, tasks, explain if needed.
• What did you see, hear, feel (lighting changes, vibrations, heat, cold, wind etc)?
• What were the environmental conditions (weather, light, noise, etc.) at the time?
• What was (were) the injured/involved worker(s) doing at the time?
• In your opinion, what caused the incident?
• How might similar incidents be prevented in the future? What would you (witnesses or involved workers) do differently?
