Dr.  Dan Shapiro is a plastic surgeon certified by the American Board of Plastic Surgery. He has been practicing cosmetic or aesthetic plastic surgery in the Scottsdale Paradise Valley area since 1992 and all he performs is cosmetic or aesthetic plastic surgery.

Dr. Shapiro does a lot of tummy tucks. He does a lot of body contouring surgery, and a common question that he gets, you know, from people that are coming in and asking about that is: 'Dr. Shapiro, what is the pain going to be like after a tummy tuck and when can I walk around?' The answer to the question is - it is revolutionized, the pain relief has been revolutionized in the last 5 to 10 years. We think in the past, you know, tummy tucks were excruciating in terms of discomfort. The reason is we will almost always plicate, or sew the muscles together, to make them very tight and give you more of a curvy waistline, a smaller waistline that accentuates your curves.

With that tightening comes discomfort. So, prior to about two-three years ago, we were using a pain pump, which actually would have catheters that go into the incision site and give you medication for the first week. Over the last two or three years there's been a new product on the horizon and we've been using it for about a year now. It's called Exparel, and it is a long-lasting injection that we use in the operating room to areas where we have tightened up the muscles and it virtually makes patients pain-free for the first four or five days. That really is the initial period where people are very uncomfortable.

Why is that important? Because pain relief means that you can get around more easily, it means you can take deeper breaths, avoid complications like atelectasis or pneumonia, and overall it makes you much more acceptable to being active. It makes you more acceptable to having a better outlook after surgery, and overall makes you feel better.

We still give people pain medication and some Valium to help the muscle relax, but with Exparel we think it's very important that you're every comfortable and it's really a winner. The other thing is, in terms of getting around, we actually get our patients up and around the day of surgery. That important because the number one risk after a tummy tuck is a blood clot, so we don't want the blood to pool in your legs. During surgery and then after, we will give you something to thin your blood, and that is called Lovenox - it's a Heparin derivative, and also during surgery there are little compression boots that go on the legs that help you elaborate your own blood thinner.

Because of that, we will get you up and around. We want you active, by the second day you're walking around, you know, three-four times a day, and then you start getting more and more active over time. Within the first week to ten days you will have some drains in that make you feel like 'I'm recovering from surgery' but since you're more comfortable, since you have much less pain and discomfort, we think you're more able and more enthusiastic about getting around.

Oftentimes with our patients we have to hold them back; they feel so good we need them not to do so much. So you know, tummy tuck and body contouring surgery has been revolutionized by this pain control that we're able to offer now. We offer it to all our patients. We think it makes you happier, it makes you much more comfortable. We think it decreases the complication rate, at least in my patient population, and overall we think it makes you a much better and happier patient.
