Approach with science and logic enlarged pores can be predictably treated with lasers, chemical peels, topical medications, & systemic medications.

Firstly let us explore the cause of enlarged pores- it can be due to genetics, modified with age. For patients over the age of 20 there is a yearly decrease in collagen. Collagen as we know supports the dermal aspect of skin including pores. As collagen decreases after the age of 20, pores maybe more visible. Genetics also control the size of pores. It is well known that patient of ethnic extraction including Asians, will often have larger pores compare to their Caucasian counterparts. Part of this is the genetic variance due to adaptation mainly cooling.

Pores can also enlarge with excess oil production. With high flow oil glands, the entrance of the pore itself enlarges. Other factors which can in large pores include UV damage. Ultraviolet a ultraviolet a penetrate into the dermal aspect of skin breaking down collagen and elastin which are key component in structural support of pores

Treatment is as follows-
Number one reduce the amount of oil production. This can be done with topical retinoids or oral retinoids. Anti-hormone medication including spironolactone can be highly effective

Increase support of the hair hair follicle or oil gland.  This can be achieved through topical skincare however the results from fractional Iasers  are more predictable. Patients of ethnic descent are streamlined to fractional picosecond lasers.

Chemical peels including TCA, salicylic acid, & AHAs acids including lactic and glycolic acid can improve collagen generation and hence provide more structural support for in large pores

The most important aspect is to understand is that we will shift the size of pores to the left of the bell curve, namely to reduce sizing based upon where you lie on the bell curve. With a strategic and analytical approach, we can help the majority of patients within large pores. For patients with BDD or body dysmorpobia, a psychologist or psychiatrist will be more effective than a dermatologist.
