Following tooth extraction or tooth loss, alveolar ridge remodeling occurs as the bone heals. This remodeling appears to be a consequence of the adaptation of bone mass and structure to the new levels of strain after extraction. The remodeling has been shown to be more pronounced buccally than lingually, which often results in a sloped ridge.

An implant can also be placed into a fresh extraction socket. However, studies have shown that despite an implant in place, similar remodeling of the alveolar crest occurs, resulting in a sloped ridge in a lingual to buccal direction.

In these situations an ideal solution would be to use an implant with a sloped shoulder that follows the bony anatomy after remodeling, optimizing the bone support and, thereby possibly avoiding the need for augmentation procedures.
