Sometimes we transfuse blood to these patients on a hemodialysis at the initial stage when haemoglobin is very low which cannot be bought by whatever the EPO available in the market and with the patient with a complaint who follows regularly with the nephrologist their haemoglobin even can be maintained from the CKD stage 1 or 2 or 4 too. Once they go on dialysis it can be followed with drugs. But some patients who do not follow their haemoglobin will be pretty low on initiation of dialysis. That moment of time definitely they require a blood transfusion to bring up the haemoglobin immediately and later on to start them on erythropoietin. Normally available in the market is the erythropoietin as well as the darbepoetin which is little long acting compared to erythropoietin which is short acting and both have the same advantage and disadvantage where it as an improving the haemoglobin. Other causes of anaemia in dialysis is basically iron deficiency. People having a kidney failure intake is very poor and their absorption of iron is very low. So hence we have to correct that iron before starting on the erythropoietin or the darbopoietin to act on that. Then only the haemoglobin can be maintained and normally maintained between around 11 to 12 in these patients because more than 12 again it can cause hypertension and increased morbidity. We cannot maintain them as a normal population. To some extent they will get a good quality of life to maintain haemoglobin and other causes like vitamin B12 have to be ruled out and if they have any infection in their body the EPO will not respond in them. Hence we have to treat some infections and treat them properly at the initial stage and then take them forward on with erythropoietin.
