The screwmentable hybrid technique is
The screwmentable hybrid technique is an affordable method of fabricating an accurate and completely passive fitting hybrid restoration on multiple implants.
Remove the healing abutments. Insert multi-unit abutments with a hex driver. Insert impression transfers and then secure them with impression transfer screws. Take an open tray impression of the arch. Once the material has set, remove the impression transfer screws and remove the impression.
Remove the impression from the mouth and attach multi-unit analogs to the impression transfers using the impression transfer screws.Flip the impression and place analog multi-unit abutments into the impression transfers.
Place soft tissue cast material around the analog platforms in the impression and then pour the rest of the impression in stone. Use wax and denture teeth to create an ideal setup on the cast. Create a putty matrix of the tooth positions to aid in fabrication of the cast bar. Remove the tooth setup from the cast and begin fabrication of the bar.
4 separate CAL components: Multi-unit coping, Spacer sleeve, Casting sleeve and Multi-unit abutment screw.
Place Multi-unit copings onto the stone model and secure with Multi-unit abutment screws. Place the Spacer sleeves over the copings. Next place the Casting sleeves over the Spacer sleeves.
Place the putty matrix onto the cast and evaluate the positions of the Screwmentable components........adjust the coping and sleeves as necessary to fit within the confines of the tooth setup.
Apply casting resin such as Primopattern to the outer waxing sleeves and begin fabrication of the bar. Cut retention holes into the teeth and add extensions to the casting pattern that will fit into the retention holes for added strength.
The resin bar pattern is now complete. Remove the spacer sleeves from the framework......this will create a uniform cement space to pick up the cylinders later
Send the resin pattern to a lab to be cast in metal. Place the metal copings back into the mouth with the multi-unit coping screws. Verify the passive fit of the bar in mouth.There should be a uniform cement space between the frame and the coping.
Remove the metal bar and apply resin cement to the copings.
Place the bar back in the mouth. Once the cement has dried, remove the metal bar unit from the mouth.
Process the denture teeth and gums onto the bar with your method of choice. Place the completed screwmentable hybrid bridge in the mouth and torque the multi-unit coping screws to 15 Newton Centimeters.
