There is a classical bow leg which happens when the child is growing, which is basically because of deficiency of Vitamins. In there, you have the bow legs where the leg bones are bent and they are bowed. And that needs a different kind of correction which needs to be done where you have to correct the bones and that is usually done by our orthopedic colleagues. But most commonly when people talk about bow legs, it is when the women are wearing skirts, they see that their legs are very thin and they have the outer curve, but the inner curve is missing. So that is what is commonly called as bow legs. So when that kind of a cosmetic bow leg deformity is what we are talking about, then we have an option for it. It is basically because of the underdevelopment of the muscles on the leg. The inner muscles or the calf muscles, the gastrocnemius muscles and the soleus muscle, that is what gives you the inner curve of the leg. When that is missing and only the outer curve is present, then the leg appears bowed which is actually not a bony problem, but it is a soft tissue problem. When that is the case, to give you a more normal look and to equalize the legs, then what we do is called a calf augmentation. Augmentation meaning, the calf muscle of the leg, the inner side is missing and the outer curve is present, so we have to put something in to create the inner curve. How is this done? There are two ways to do it. One is you can either use your own fat, which is called an autologous fat grafting, where we use the fat and inject into the leg to give you the proper shape and the second is the use of implants. What are these implants? These are basically silicon gel implants that we use. We don’t use the solid implants here. The silicon gel implants are inserted by a small 5 cm cut behind the knee, within the crease or the fold, so that the scar merges within the fold. From there, the implant is inserted to give a nice curve. And thus, the implants are usually permanent, you don’t need to change them, unless of course there is an injury or a rupture. It is a very simple procedure. It is a very satisfying procedure for the patients and gives you a more normal look to the legs. It is normally done as a daycare procedure. Only contraindication or precaution being, for the next 3 weeks we have to use heels to walk, so that the muscle is relaxed and the implant settles in properly and once its done, it gives you a very aesthetically normal looking leg.
