CASE 1. Corrective Jaw Surgery. Introduction to Orthognathic Surgery.
Correction of facial proportions, advancement of airway and correction of class II malocclusion, achieved with counterclockwise maxillomandibular advancement. Total advancement of approximately 34 mm at the pogonion. Chin lengthening operation is essential to balance the lower face. HA cheek implants are used to further define malar region.
Deep labiomental crease is hard to correct , HA bone graft material may be used to plump the crease up.
The most important steps in post op care of big advancements like this are:
stable condyles.
class II elastics to keep pressure away from condyles.
skeletal suspention with vertical elastics to to avoid skeletal relapse.
prefer to use TADS ( bone screws) for skeletal traction.
