In and around the knee joint, you may find several types of cysts.
• Ganglion cyst of the ACL
• Meniscal cyst
o True meniscal cyst
o Baker's cyst
 Further from the meniscus

Ganglion cyst of the cruciate ligament (ACL)
Ganglion cyst of the cruciate ligaments are rare.  The ACL ganglion cyst is identical to the ganglion cyst that are seen everywhere.  It may result from mucoid degeneration of the ACL.  Ganglion cyst involving the cruciate ligament can cause pain in the knee and may interfere with range of motion.  The patient does not have instability.  The symptoms are usually poorly localized in the differential diagnosis of this entity should be a torn medial meniscus.  The patient may have symptoms of catching, but not locking.  What do you see on the MRI that involves the ganglion cyst of the cruciate ligament?  The MRI will show a cystic structure within the anterior cruciate ligament.  You will see a fluid-filled lesion with an increased signal in T2.

Treatment
• Usually conservative
• If conservative treatment fails, then arthroscopic debridement of the cyst can be done.

Differential diagnosis for ganglion cyst of cruciate ligament
• medial meniscus tear
o We will check medial joint line tenderness.
o Check McMurray test.
o Check the MRI findings
• Baker's cyst
o Usually, it is posteromedial and it presents between the medial head of the gastrocenemius and the semimembranosus.
• Meniscal cyst
o The meniscal cyst occurs in the middle third of the lateral meniscus and it occurs due to the meniscal tear.
o The meniscus will have a tear and there is a small amount of fluid within the meniscus, and the fluid will go through the meniscus and will make a cyst next to the meniscus.
o It is usually seen in a horizontal tear or a horizontal cleavage tear.
o Treatment: If the tear communicates with the joint, you can do an arthroscopic partial meniscectomy and cyst decompression.  If the tear does not communicate with the joint, then you do arthroscopy and you may want to do an open procedure for the cyst.

• Baker's cyst
o The fluid is getting extruded outside of the meniscus and makes the cyst usually behind the knee.
o Baker's cyst is sometimes called a "popliteal cyst ".
o A Baker's cyst is a benign swelling found behind the knee.
o The cyst is connected to the knee joint through a valvular opening
o The Baker's cyst is filled with fluid that is coming from inside the joint, usually due to intra-articular pathology inside the joint that causes filling of the joint with fluid and pouring of the fluid into the cyst.
o The fluid will go through the valve to the cyst in one direction.
o If the patient presents with a Baker's cyst, then you need to think about problems inside the knee that caused that Baker's cyst.  
o The cyst is usually at or below the joint level.
o The patient will complain of pain, fullness, and tenderness.
o It is easier to see the cyst when the knee is extended.
o MRI will show the cyst and it will also show the associated intra-articular pathology.  The intra-articular pathology is usually either knee arthritis or a meniscal tear, especially the posterior horn of the meniscus.
o If the cyst is present in an atypical location, then rule out a tumor or another differential diagnosis.
o The Baker's cyst fluid filled sac will transilluminate (it is not a solid tumor).

Baker cyst treatment:
• Ice
• Anti-inflammatory medication
• Physical therapy
• Aspiration of the cyst
• Excision of the cyst may be needed
o Recurrence of the cyst is common if the intra-articular pathology continues.  The best treatment for the Baker's cyst is arthroscopy and debridement of the intra-articular pathology (torn meniscus).
