In this video, Dennis Hartlieb, DDS, AAACD, demonstrates the steps to perform a freehand diastema closure with resin using direct composite bonding.

Preparation
First, Dr. Hartlieb measures the centrl incisors vertically and horizontally to determine their proportions. He polishes the teeth with pumice to create a clean surface and prepares the margins with retraction cord. Dr. Hartlieb then uses a Brasseler 6862-012 bur to run over the surface of both incisors and refines the surface with the 889M007 bur.

. Hartlieb isolates tooth #9 with Teflon tape and a matrix band. He applies etching gel to the tooth for 15 to 30 seconds and removes it with water. Next, he applies the dental adhesive and light cures it.

Applying the NANO Plus Layer Foundation for Strength
With the tooth prepped, Dr. Hartlieb adds Renamel NANO Plus. He rolls a small amount into a thin coil and wraps it along the incisal and proximal edges of the tooth. A small amount of the nanohybrid is spread across the facial surface, blending the material together using the IPCL instrument. Dr. Hartlieb uses a gauge to measure tooth #9 and adjusts accordingly by adding or subtracting NANO as needed. He then light cu
res the material.

Dr. Hartlieb isolates tooth #8 and repeats the etching, adhesive, and NANO applications. He applies Creative Color White Opaque to the incisal edges of both teeth and light cures the opaquer.

Microfill Application to Replicate Enamel
Microfill Incisal Light is applied from the incisal edge and blended up to the bottom third of the facial surface on both teeth. Microfill A1 is applied and blended down from the cervical towards the middle third. Microfill B0 is then sculpted across the entire facial surface. Dr. Hartlieb uses the IPCL to spread the material, even out the surfaces, and remove excess. The microfill is light cured between each added layer.

Finishing and Polishing the Diastema Closure
After light curing, Dr. Hartlieb uses an OSI bur along the incisal edges and an ET9 bur across the lingual surface to start the finishing process. He runs an 8392-016 bur across the cervical portion of each tooth. Next, he removes the retraction cord and uses a FlexiSaw and Fine FlexiStrip to polish the contact. Dr. Hartlieb then runs both the Medium and Superfine FlexiCups over the surface.

A Medium FlexiDisc and 135F bur complete the finishing and shaping. Dr. Hartlieb applies a FlexiBrush point and the Fine and Superfine FlexiDiscs to start the polishing process. The final polish is achieved with Enamelize Polishing Paste and a FlexiBuff disc.
