Steps to apply for online name change in Gazette
1) Go to https://dgps.maharashtra.gov.in
2) Go to name change section as shown in video
3) If already registered, enter username and password and click login. If not yet registered, go to new user and create new username/password
4) Once logged in, go to Name change option
5) Read the disclaimer
6) Download the form. Print this form. This form has to be filled and signed.
Details like Current name, Changed name, Address, Reason for name change has to be filled.
Scanned copy of the this form has to be uploaded in proof upload section
7) Enter all the details in form
8) Submit the form and verify the entered data
9) Upload all relevant proofs as below
1) ID proof
2) Address proof
3) Latest photograph
4) Filled and signed form
5) Supporting document for name change reason
10) Make the the payment
11) Note down the Application id or token number for tracking.
