Duff shares his story about rising out of homelessness with help from Yakima Neighborhood Health Services. As part of Medicaid transformation, the Health Care Authority's Foundational Community Supports (FCS) program uses Medicaid dollars to help fund case management services for high risk individuals needing support in finding a job and a place to live.
