Numerical on PD/PID Controller to Find Controller Output (Parallel form)

How to find slope:
At time period 3 to 5 are x1 = 3 , y1 = 1, x2 = 5 , y2 = 0
slop = [(y2-y1)/(x2-x1)]
         = [(0-1)/(5-3)] = -1/2 = -0.5 %/seconds
