The Swiss cheese model of
The Swiss cheese model of accident causation is a model used in risk analysis and risk management to describe the different barriers in place to prevent an event from occurring, and also the inherent weaknesses of each barrier. This tool is used to show how multiple barrier failures align to allow an incident to occur.
