A postmenopausal woman at the age of 56, with a family history of ovarian cancer in her sister, has been experiencing abdominal pain and gastrointestinal discomfort for the past three months. Initially, I would conduct a physical examination to check for signs of ascites, palpable lumps, breathlessness, or palpable supraclavicular lymph nodes.

Subsequent sonography revealed the presence of bilateral ovarian masses along with ascites, while a CT scan indicated bilateral ovarian masses, peritoneal nodules, ascites, and omental deposits. These findings suggest a probable stage 3 ovarian cancer. To determine if the disease has spread further, potentially reaching stage 4 by involving the liver or lungs, further evaluation such as a chest CT scan or PET scan would be advisable.

Following this, an endoscopy would be performed to examine the abdominal cavity, and a biopsy would be taken from the tissue samples to confirm the diagnosis.
