The Nernst Equation is used to determine the cell potential for an electrochemical cell prepared using the SAME metal electrodes & the same metal in solution, but the dissolved metal is at DIFFERENT molar concentrations.  Since the concentration difference is the driving force (any gradient will cause "the universe" to proceed in one direction or the other), electrons will flow so that Equilibrium will be established (as the universe does), and the more concentrated half cell will become more dilute, so the reaction simplifies to Concentrated metal Reactant becomes the more dilute metal Product.  This is important when establishing Q, the reaction quotient, which is always Products/Reactants (at any time during the reaction.)
