Audience: Medical Interns and Residents: Surgery, Emergency Medicine, Radiology
Learning Objectives:
Differentiate pseudoaneurysm from active extravasation
Recommend the next step in management based on key imaging features
Summary:
Pseudoaneurysm
-Follows arterial blood pool
-Does not change shape
Active Extravasation
-Remains dense despite some degree of dilution
-Expands or pools
Is the patient hemodynamically stable after resuscitation?
-Attempt to manage non-operatively
-CT with arterial, portal venous, and delayed phase
Is there pseudoanuerysm or active extravasation?
-Treat vascular injuries with angiography
Higher grade injuries don’t always need surgery, but
-Greater chance of delayed complications
-Consider Follow-up imaging in 48 to 72 hours
