The ulnar artery is a major vessel of the upper limb that supplies blood to the forearm and hand. Understanding its anatomy, course, branches, and clinical significance is crucial for medical students preparing for the USMLE exams. This article provides a concise overview of the ulnar artery, focusing on its course, branches, and relevant clinical aspects.

Course of the Ulnar Artery:

Origin: The ulnar artery arises from the brachial artery in the cubital fossa, which is located anterior to the elbow joint.
Pathway: From its origin, the ulnar artery descends along the medial aspect of the forearm, running parallel to the ulnar nerve. It passes behind the medial epicondyle of the humerus and enters the forearm between the two heads of the flexor carpi ulnaris muscle.
Course in the Forearm: Within the forearm, the ulnar artery runs deep to the flexor muscles, coursing along the ulnar side (medial) of the flexor digitorum superficialis muscle.
Course in the Hand: At the wrist, the ulnar artery enters the hand through the Guyon's canal, a passageway formed by the pisiform and hook of the hamate bones. It gives rise to several branches that supply blood to the hand.

Branches of the Ulnar Artery:

Common Interosseous Artery: Shortly after entering the forearm, the ulnar artery gives rise to the common interosseous artery, which further divides into anterior and posterior interosseous arteries. These arteries provide blood supply to the muscles in the anterior and posterior compartments of the forearm.
Muscular Branches: Along its course, the ulnar artery gives off muscular branches that supply blood to the flexor muscles of the forearm.
Ulnar Recurrent Artery: Near the elbow joint, the ulnar artery sends a small branch called the ulnar recurrent artery, which anastomoses with the radial collateral artery. This anastomosis forms an important collateral pathway in the event of occlusion or injury to the radial artery.
Superficial Palmar Branches: In the hand, the ulnar artery gives rise to superficial palmar branches that form the superficial palmar arch. This arch gives off digital branches that supply blood to the medial two and a half fingers (little finger, ring finger, and half of the middle finger).

Clinical Significance:

Ulnar Artery Occlusion: Occlusion or blockage of the ulnar artery can lead to ischemia (lack of blood supply) in the forearm and hand. It may present with symptoms such as pain, pallor, coldness, and decreased sensation in the affected area.
Allen's Test: The ulnar artery plays a role in assessing collateral circulation in the hand. The Allen's test is performed by compressing both the ulnar and radial arteries at the wrist while the patient clenches and releases their fist. Release of pressure on the ulnar artery should result in prompt filling of the palm, indicating adequate collateral circulation via the ulnar artery.
Ulnar Artery Cannulation: The ulnar artery can be used for arterial cannulation in certain clinical scenarios, such as continuous blood pressure monitoring or arterial blood gas sampling.
