Time for another Cataract Quiz: You are doing phaco for this patient with a traumatic white cataract and everything seems to be reasonable. A good capsulorhexis with adequate zonular support and good tissue response to phaco chop. But when you remove some of the nuclear fragments you notice that there is no red reflex. The posterior segment of the eye looks dark no matter how you adjust the microscope lighting. Even with the nucleus fully removed, the cortex aspiration, and confirmation that the capsular bag is intact, there is still no red reflex. Why? Comment below before watching the video.
