Here is a patient who underwent surgery for a right medial patellofemoral ligament (MPFL) reconstruction. A consequence of this surgery is disuse atrophy of the quadriceps. The lack of quadriceps mass and strength significantly limits the functional independence with walking, squatting, and negotiating stairs. It is crucial to minimize atrophy while also being mindful of protecting the integrity of the surgical reconstruction/repair.

Her tolerance for weight-bearing tolerance improved every week, in addition to decreased pain and swelling, as well as improved knee range of motion (ROM) and quadriceps activation.

This is her first time performing leg press.  There is minimal valgus collapse with this exercise.  It is easier, and safer, to teach this movement pattern (hip and knee extension) in a gravity-reduced position, as opposed to squatting, especially single-leg squatting.

The first exercise is double leg press which is relevant for sit to stand/stand to sit.  Single-leg press is the second exercise and is relevant for walking, and going up and down stairs.

She is using the Blood Flow Restriction (BFR) bands while performing these exercises. Training with BFR bands creates an anabolic environment from a strenuous workout while only using a light resistance load. Tissue repair is facilitated resulting in improved strength and muscle mass without damaging the repair.

Effective and efficient rehab is criterion-based, not time-based.

Always consult your physician before beginning any exercise program.  STOP exercising if you experience any symptoms of weakness, unsteadiness, light-headedness / dizziness, chest pain or pressure, nausea, or shortness of breath.  

Performing these exercises should not cause pain.  If you experience pain while exercising, decrease the speed of your movement.  If the pain persists - STOP.  Mild soreness after exercising may be experienced after beginning a new exercise.
