This is part 1 of 3 of a great case that perfectly represents multiple presentations I discuss all the time. First of all we always try to get out all the cyst contents, however some abscesses are so painful the patient can not tolerate any more, so you get out as much as you can and let your prescriptions alleviate the inflammation over several days. In a case like this, draining the contents will alleviate the pressure which will greatly reduce the patients pain quickly. Approximately 50% of these lesions do not have bacteria driving the inflammation. This one did and I will discuss that in the next video why we had to switch antibiotics and the importance of culturing the wound for bacteria.

Second, this is a perfect example of the acid base shift that happens in a severely inflamed wound. The wound becomes very acidic which affects the ability of lidocaine to anesthetize the area. I infused 13-15cc into this wound (double what I normally need) and the max of what is recommended. Lidocaine is an acid, so applying an acid to an acidic environment it will not work as intended. Important to note the bacteria infected inflamed cysts tend to be much more painful than the ruptured cyst abscesses that have no bacteria. The reason for that is bacteria drives the bodies inflammatory response into hyperdrive making a more acidic wound.

Third, I always talk about cysts that have broken down inside will eventually reform and need an excision when all the inflammation has resolved. Video number 3 will showcase his excision and follow up. He was so happy to have all of this taken care of. There was a language barrier, however his family member told me how thankful he was for me to do this because an urgent care/ER provider told him they couldn’t help him. It was too big and his pain wouldn’t let them proceed.
